Filling out the form does not schedule a vaccination, but will help collect contact information as vaccination opportunities arise.
Friday, January 22, 2021  ::   Oak Park is set to move into Phase 1b of the state’s plan for COVID-19 vaccinations next week. Some 12,000 residents and essential workers who live or work here are eligible, including those 65 years old and older. Vaccine supplies will determine the timeline.
