Community Health Needs Assessment

Every five years the Oak Park Department of Public Health asseses the community's public health, its needs and available resources, then creates a community plan for how best serve its constituents.

This Community Health Needs Assessment is part of the process of maintaining the Oak Park Department of Public Health's state certification. The Oak Park Health Department is one of only a handful of local health departments certified by the Illinois Department of Public Health.

A community health needs assessment includes a community survey, an assessment of the Health Department's capacities, collection and analysis of available public health data, and meetings with stakeholders such as hospitals, fire and police departments, and senior administrators of the villages of Oak Park and River Forest.

For 2017, the Oak Park Health Department has partnered with the Community Mental Health Board of Oak Park Township, the River Forest Township Mental Health Committee and the Rotary Club of Oak Park and River Forest.

The first phase of the process is the community survey, which is currently underway through April 15, 2017.  Anyone who lives, works or attends school in Oak Park and River Forest is invited to take the online survey at  at
